Tom was working on a training assignment on Cat 777B dump trucks at the Bogasu Gold Mine in Ghana, West Africa around 1991.
This Caterpillar D7H was feeding a 988B loading shovel from the upper plateau at the time of the incident; unfortunately the shovel operator had seriously undermined the face, causing the dozer to fall due to its own weight.

Tom said, “You could not have planned this if you tried, incredibly the dozer simply landed on its blade and we just hooked a chain on it and pulled it upright again!”
